Waiver consent:
Enrollment in Soccer Fun Camp programs permits my child/player to participate in any/all program activities. I assume all risks and hazards incidental to program participation. I do here by waive, release, absolve, indemnify and agree to hold harmless Soccer Fun Camp personnel, sponsors and coaches for any claim arising out of any injury to my child. I also understand that it is my responsibility to notify the coach of any medical/physical condition(s) that could limit my child’s/player’s participation or special attention.
Media consent:
I, the undersigned parent or guardian, do authorize Soccer Fun Camp to utilize any and all photographs for or as deemed necessary for our websites and other promotions.
Refund Policy

30+ days notice (from first day of camp) - full refund minus $35 registration fee or voucher for another camp of equal or lesser value.

14 to 30 days notice, full refund minus $50 reg. fee OR $30 fee plus voucher.

7-14 days notice - full refund minus $75 fee OR $40 fee plus voucher.

Less than 7 days notice will receive 50% refund OR $50 fee plus voucher.

Vouchers good for 12 months.

No refunds for no-shows and leaving early.
